Saying Bless You Heart is not good. It is a subtle dig. I think Justin was not being kind to Johnson, rather saying what you said, Johnson’s got no heart.

Same in Utah - "bless your heart" is basically saying "you're a doofus" - used in place of rolling your eyes, though we will sometimes do both

I learned about that little dig after living in Georgia. Like you mentioned, it means something else other places. I have enjoyed adopting the Southern version.
